Service Coordination

By working one-on-one with an Independent Living (IL) Services Coordinator, you can develop a plan of action for increasing your level of independence. You maintain full control over your plan, know­ing that your IL Services Coordinator is there to answer questions and make recommendations when you need support.

Who benefits from IL Services Coordination?

Individuals who could benefit from working with an IL Services Coordinator include those who:

·        Are able to self-direct and express a desire to increase their independence

·        Have a physical, mental, cognitive or sensory disability that has significant impact upon their life

·        Are substantially limited in their ability to function independently
in the family, community or vocationally because of their disability

How an IL Services Coordinator can assist you

Think of your IL Services Coordinator
as your first resource for achieving your independent living goals. If you are eligible for Services Coordination, your IL Services Coordinator will:

·        Talk with you one-on-one, either
in person or on the phone

·        Listen to your needs

·        Listen to your short- and long-term goals

·        Document your stated goals in an Independent Living Plan

·        Refer you to people and services that can help you achieve your goals, both at Granite State Independent Living and other agencies

·        Act as a sounding board as you work toward your goals

·        Help you modify your plan when you meet goals and when you experience changes in your life

·        Model independent living skills and attitudes

·        Work with you as long as you want to keep your Independent Living Plan active

Who controls IL Services Coordination

You do. Throughout the Services Coordi­na­tion process, you are involved in all aspects of developing and implement­ing your IL plan. Only you have authority to set, change or drop IL goals.

Who are Services Coordinators?

Services Coordinators are people who have had or are currently experiencing disability issues. They are trained to guide other people with disabilities through the goal-setting process and through the maze of supporting services available throughout the state.

What are independent
living goals?

Independent living goals can be any­thing you define as important in living your life on your terms. They could include any of the following goals:

·        Vocational

·        Activities of daily living (ADL) skills improvement

·        Financial

·        Social

·        Recreational

·        Access

·        Other self-defined goals

Benefits of IL Services Coordination

Individuals can:

·        Set their own goals

·        Completely manage their own plan

·        Work on goals that are not connected to medical service or that rely on a diagnosis

·        Share ideas with their IL services coordinator

·        Gain referrals to services

·        Gain peer interaction during meetings and planning sessions

·        Work with IL services coordinators that have had or are experiencing similar disability issues

A free service

IL Services Coordination is free for eligible individuals. In addition, the following are NOT required:

·        Medical diagnosis

·        Specific levels of ability

·        Financial need

·        Insurance coverage

How to access IL Services Coordination

If you are interested in working with a Granite State Independent Living Services Coordinator to set and achieve your independent living goals, contact Granite State Independent Living by phone or at info@gsil.org. An IL Services Coordinator will schedule a telephone or in-person service coordination meeting with you to determine if you are eligible for services.
