Personal Care Service Provider Services

If you, or someone you love, are living with a disability, a Personal Care Service Provider (PCSP) may be just what you need to gain or maintain your independence and thrive in your community.

PCSP services follow a Consumer directed, person-centered approach to addressing personal care and community access needs under the Choices for Independence (CFI) waiver through Medicaid. The PCSP care plan is developed with the Consumer and outlines the number of authorized personal care hours available to the Consumer. The care plan aligns with the guidelines of CFI’s allowable tasks, while focusing on the individual’s unique needs and preferences.

Enrollment Eligibility

• Must be a minimum of eighteen (18) years of age.
• Must have authorization for PCSP services under CFI from a case management agency.
• Must have the ability to meet the responsibilities of consumer-direction.
• Must need hands-on assistance with non-medical tasks that allow the consumer to live independently.

Services Provided

The PCSP program offers a variety of non-medical assistance services, each tailored to support the Consumer’s unique needs and promote independence in their home. By focusing on personal care, daily living tasks, and community access, the program aims to ensure that each Consumer can engage in activities and routines that promote their quality of life with independence.

Each Consumer is assigned a Long-Term Supports Coordinator who may help with recruiting, interviewing, hiring staff, identifying community resources, and developing personal goals.

PCSP services may include assistance with:

• Bathing
• Hydration/Nutrition
• Errands
• Dressing
• Meal/Snack Prep
• Toileting
• Grooming
• Service Animal Care
• Transfers/Mobility
• Light Housekeeping
• Range of Motion Exercises
• Laundry
• Grocery Shopping
• Medication Reminders

Consumer-Directed

A Consumer-Directed program is designed to empower the Consumer. The Consumer will act as a co-employer with GSIL and will directly supervise the Attendant Care Employees (ACEs).

The Consumer will:

• Assist in creating their own Care Plan and Independent Living Plan.
• Set the schedule for ACE(s).
• Develop an emergency backup plan for care.
• Review the bi-weekly timesheet approval report to verify worked hours by ACE(s).
• Abide by GSIL’s policies and procedures.
• Screen, interview, and hire ACE(s) with help from the Coordinator as needed.
• Support ACE(s) in completing and submitting new hire paperwork.
• Train and supervise ACE(s).
• Ensure a safe work environment for the ACE(s).
• Maintain face-to-face contact with the Coordinator at least four (4) times per year.
• Stay in contact with the Coordinator at least every sixty (60) days.
• If needed, choose an Authorized Representative or Personal Care Representative to assist with managing services.
• Report any changes to the Coordinator immediately.
