Attendant Care Worker Training Form – Any training conducted by the supervisor will be recorded on the Attendant Care Employee Ongoing Training Form which will identify the following:

1. Name of employee/attendee.
2. Date of Training
3. Duration of training
4. Name of trainer/presenter
5. Training topic as well as a brief description of the training.

The supervisor providing the training will complete the training form. The form will be sent to HR assistant to be filed in the records.
